Home Visit
A member of the Divine Care team will visit your home to undertake an assessment of care and risk. We will design and cost your care package following this visit.

Enjoy Our Care Plan
If you accept this care package, we will finalise your care plan with you, in conjunction with healthcare professionals and / or your family if required.

Once your care commences, the Registered Manager will visit you on a regular basis to make sure we are still providing you with an excellent service and that we are meeting your needs.
